When registering for a conference, please put the name of the person attending where it says "Attendee's First Name" and "Attendee's Last Name." If a company card is being used for payment, be sure to indicate the billing address is different than the attendee's address. Also, remember there will be opportunity to indicate the names of additional registrants and guests when the Additional Comments box appears.

Please note: If you want to register more than one person online per payment, you will need to either fax a list of the registrants' names and all other pertinent information to EMLF at 859.226.0485 or complete the Additional Comments box. Up to 10 registrations can be paid for at a time with online registration - simply change the quantity when the shopping cart screen comes up and click on "Recalculate Quantities."

You will also have an opportunity to list guests and additional registrants when you submit your completed form (with name, address, etc.). Once that form is submitted, the next window includes a text block for additional comments where you can add that information.
